Both pages of the application need to be completely filled out and all additional documents along with the $25 application fee required BEFORE application can be approved.

The photo of the boat needs to be recent and in color. All documents may be emailed or faxed, and the Application fee needs to be in either cash or check and can be mailed or brought in person.
